Contract Health Services
The Cow Creek Contract Health Services Program is a federally funded program, contracted through the Indian Health Services. The program is designed to assist eligible Tribal Memberss with health coverage to provide medical services, as well as mental health eye exams/glasses and a dental benefit.
To be eligible for this program, applicants must be a tribal member with the Cow Creek Band of Umpqua Tribe of Indians, have submitted a completed 2010 Health Application with a determination of eligibility, and reside within the IHS mandated seven county service area. Once eligible, each date of service requires a purchase order (PO) number in advance. All purchase orders are date sensitive, therefore, if the Tribal member reschedules an appointment, they must call a CHS representative to obtain a new purchase order.
Any type of surgery, MRI or Sleep Study requires prior approval through the Resource Management Committee. Tribal members must contact CHS at least 1 week prior to the appointment.
