We know that you, as a patient, are much more informed about your health, and want to have an active role in decision-making and options regarding your care. We are ready to deliver this new approach to healthcare and education by working in Medical Care Teams.
We have four Medical Care Teams and specialty staff providing you high quality care. Together, you and your team will get the needed support by experts and leading healthcare professionals. All of our Medical Care Teams rotate between our North Roseburg and South Canyonville clinics so that you can see them at either facility.
Tribal clinics across America have taken the lead in transforming the way healthcare is delivered to their patients. Tribal Healthcare for the Cow Creek Band of Umpqua Tribe of Indians began in 1982 and consisted of primarily home health visits and the purchasing of outside services with Purchased and Referred Care (formerly Contract Health Services) funds. The Urban Indian Clinic, located in Portland, Oregon, also served to provide healthcare to Tribal members.
In April of 1999, the Cow Creek Health & Wellness Center first began operations with eight staff members. For over a decade, the CCH&WC has expanded to over 50 staff members and a full array of healthcare services. In 2013, the clinic grew again, building another medical campus in Canyonville, Oregon.
Our mission is to contribute to the overall wellness of individuals, families, neighborhoods, and communities throughout individualized, empowering, and compassionate care to our members.
The vision of the Cow Creek Health & Wellness Center is to work in partnership with the people we serve to elevate their physical, mental, social, and spiritual health to the highest level.
The Cow Creek Health & Wellness Center is accredited by the AAAHC (Accreditation Association for Ambulatory Health Care). Additionally, we are certified by the state of Oregon for alcohol & drug treatment, and have been recognized at both the state and national level for our exceptional diabetes care. Q: What is a Medical Care Team?
A: A team consists of a medical provider, registered nurse, certified medical assistant, patient care coordinator, and YOU. You were assigned to the team you’ve seen the most the past year(s).
Q: I want to be seen at the same clinic by the same provider every time. Is this possible?
A: Yes, your provider and team work at both clinics each week. Please call the clinic and ask when your team will be at that clinic of your choice.
Q: If I am sick and my team has no open appointments that day, can I see another provider?
A: Yes, you can call the clinic anytime to be seen if you are ill. Each team has 3 open “same day” appointments. If your team is full for the day, you can be seen by any other team with an open “same day” appointment.
Q: Is it easy to get a referral to a specialist?
A: Yes, we have several specialists and referrals staff in-house. Outside referrals are easily obtained by your healthcare team.
