Healthcare for the Cow Creek Band of Umpqua Tribe of Indians began in 1982 and consisted of primarily home health visits and the purchasing of services with Purchased and Referred Care (formerly Contract Health Services) funds. The Urban Indian Clinic located in Portland, Oregon, also served to provide healthcare to tribal members who lived in and out of the service delivery area.
The Cow Creek Health & Wellness Center (CCH&WC) first began operations in April of 1999 with eight staff members (four clinical staff and four administrative staff). Over the course of a decade the CCH&WC has expanded to forty eight positions and numerous specialty services offered in addition to ambulatory care.
In 2013, CCH&WC services were expanded to South Douglas County, in order to increase access and services for Tribal Members, affiliates, employees and their eligible dependents. It is our goal that the CCH&WC services are fully accessible, the best medical practices offered and clinic care meets the unique needs of our Clinic users. The following are the medical and educational services of the CCH&WC: four medical teams, medical nutrition therapy, diabetes prevention and treatment, tobacco cessation, retinal exams, psychiatry, podiatry, and internal medicine. Both Clinics provide lab and our south campus clinic radiology. Our tribal pharmacy is housed inside the Hometown Drug Pharmacy in Roseburg, Oregon.
We have two Wellness Centers located in Roseburg and Canyonville. Our Electronic Records system makes it possible to access clinical medical records at either location, which allows patients to access their care at either location.
Same day appointments are provided for those who are acutely ill and need an unscheduled visit. For the hours of operation, please call the main clinic number @ 541-672-8533.
