Healthcare for the Cow Creek Band of Umpqua Tribe of Indians began in 1982 and consisted of primarily home health visits and the purchasing of outside services with CHS funds. The Urban Indian Clinic located in Portland Oregon also served to provide healthcare to tribal members who were in need of healthcare and not able to access it locally.
The Cow Creek Health & Wellness Center (CCH&WC) first began operations in April 1999 with eight staff members; four clinical care staff (doctor, family nurse practitioner, nurse and receptionist) and four administrative staff (billing, CHS, CHR and Health Director). Over the course of a decade the CCH&WC has expanded both in employee number and services offered.
In late 2009, the Cow Creek Health & Wellness Center (CCH&WC) started an initiative to improve our ability to increase access to healthcare services for all Tribal members, affiliates, employees and their eligible dependents. Tribal Government has supported these efforts through their directive to open the CCH&WC Annex in Canyonville in early November, 2009 and to hire additional medical providers and support staff. We are now fully staffed with 3 female and 2 male medical providers to meet your family’s needs. It is our goal that the CCH&WC services are accessible, the best medical practices and standards and are diverse enough to meet the unique needs of our clinic users.
The Tribe is currently in the planning phaseof designing an additional 31,000 square foot health and community center that will provide the full range of healthcare services and enhance CCH&WC’s ability to provide exercise, cooking, lifestyle, evidence based education and wraparound services to support the aim of increasing the health profile of the Tribal and non-tribal communities served in the CHSDA.
Locations
All Cow Creek Tribal Indian and non-Indian employees and their families can receive healthcare services at either of the Tribe’s wellness centers as our electronic health records systems make it possible to access clinical medical records at either location.

HOURS of OPERATION
Walk-in hours are provided for those that are ill and need unscheduled healthcare services. If you are able to call ahead of time, we can schedule you a same day visit to reduce your wait time. If you are unable to call ahead of time, please note that “walk in” hours are from 11:00-12:00 PM and 1:00-2:00 PM each day.
If you or your family members are ill and not registered patients of CCH&WC, please call and come in for an initial visit with one of our providers. We will work with you to get the registration paperwork completed after your initial visit so you can continue to be seen at our facilities in the future.
